This initiative aims to improve access to evidence-based prevention and intervention programs for youth and families in rural Pennsylvania communities experiencing challenges with substance use, mental health challenges, and youth disconnection.
By integrating the Coalition Check-Up technical assistance model, communities will select strategies and programs to address community priorities. Coalition Check-Up empowers communities by putting local data into the hands of local people—giving residents, partners, and leaders the information they need to make informed decisions, strengthen collaboration, and create healthier communities.
Project Goals
- Help people in rural Pennsylvania counties learn about health resources, programs, and opportunities through clear, accessible communication and community outreach.
- Work with local community prevention coalitions to strengthen partnerships and improve how organizations work together to better meet the health needs of their communities.
- Share health and wellness programs that communities have identified as important, helping build local knowledge, skills, and resources to improve the health and well-being of residents.
By focusing on these goals, we hope to create healthier, more connected communities where people have the knowledge, support, and opportunities they need to thrive.
